Apply for Summer Program

To join the program, the applicant should complete the following required documents:
  1. Application Form (download)
  2. Scan of Passport (JPEG, see the example)
  3. Scan of statement letter for obeying UPI and Indonesian Rules (download)
  4. Soft file Photo 3 cm x 4 cm

Please submit the aforementioned documents to the host/committee of summer program. Then, the committee send it collectively by email to [email protected]

Notes for online summer program:
Item No 2, Scan of Passport can be replaced by scan of student card  or citizen card (JPEG)
Item No 3 is not required
